Clinical Causes: Trauma to the orbit (eye socket), such as fracture or injury that damages the bony structure or soft tissues. Surgical procedures involving the orbit or facial bones that unintentionally affect the position of the eyeball. Orbital fractures, particularly blowout fractures of the orbital floor or medial wall. Enlargement of the orbital cavity due to atrophy or other tissue loss, leading to the sunken appearance. Degenerative changes resulting from aging or disease that affect orbital tissues. Previous radiation therapy to the orbital region.

Key Symptoms: Noticeable sunken appearance of the affected eye, especially in the right eye in this context. Aesthetic concerns and asymmetry of the face. Possible double vision or visual disturbances if ocular muscles or nerves are involved. Restricted eye movement in some cases. Discomfort or mild pain around the affected area following trauma or surgery. Ptosis or drooping of the eyelid may sometimes occur.

Diagnosis Path: Physical examination to assess the position of the eyeball relative to the orbit. Comparative assessment of both eyes for symmetry and globe position. Imaging studies such as computed tomography (CT) scans or magnetic resonance imaging (MRI) to visualize orbital bones and soft tissues. Orbital measurements to quantify the degree of enophthalmos. Assessment of ocular motility and vision to identify any functional impairments.

Treatment Protocols: Orbital volume augmentation through surgical procedures such as orbital implant placement or fat grafting to restore the normal position of the eye. Correction of orbital fractures with surgical repairs to realign the orbital walls. Cosmetic surgeries to improve facial symmetry and appearance. Addressing any residual issues from trauma or previous surgeries that might contribute to enophthalmos. Observation in mild cases where no functional or aesthetic concerns are present, with periodic monitoring.
