Procedure Overview

Computerized tomography of the axial skeleton, excluding the skull and facial bones, creates detailed cross-sectional images of the spine, ribs, and sternum by combining many X-ray exposures taken from different angles around the body. A computer reconstructs these exposures into thin-slice images that can also be viewed in multiple planes, revealing bone detail that a plain X-ray cannot capture.

This scan is ordered when a fracture is suspected to be complex or difficult to fully characterize on plain film, when surgeons need precise anatomical detail before spinal surgery, or when a tumor, infection, or other structural abnormality within the vertebrae needs closer evaluation. It is also frequently used in trauma settings because it can rapidly and thoroughly assess the entire spine for injury after a significant accident.

The scan itself takes only a few minutes, with the patient lying still on a table that passes through a ring-shaped scanner, and contrast material may be given intravenously depending on what the physician needs to evaluate.

One recurring mistake is coding a combined CT study, such as chest CT that incidentally includes thoracic spine detail, as though it were a dedicated spine CT, when the clinical intent and protocol were actually centered on the lungs or mediastinum. Coders should also verify contrast documentation carefully, since noting "with and without contrast" versus contrast given in only one phase changes which qualifier is correct.
