Procedure Overview

Pelvic (Iliac) Veins, Right

The right pelvic, or iliac, veins carry blood from the right lower limb and pelvic organs upward to join the inferior vena cava, running alongside the right common, external, and internal iliac arteries. CT imaging of this segment is most often ordered to evaluate suspected deep venous thrombosis extending above the inguinal ligament, to assess May-Thurne syndrome in which the right iliac artery compresses the left vein, or to stage pelvic malignancy encroaching on the venous wall. Documentation should specify laterality since the right side is coded separately from the left, and the study is generally performed with contrast during the venous phase to opacify the vessel adequately, given its deep retroperitoneal location adjacent to bowel loops and the sacroiliac joint.

Contrast: Low Osmolar

Low Osmolar denotes use of a low-osmolar iodinated contrast agent during an imaging study, the class most commonly used in modern radiographic and CT imaging because it better approximates the osmolality of blood and carries a lower risk of reaction than High Osmolar media. This value distinguishes studies performed with this now-standard contrast type from those using older high-osmolar agents or no contrast at all.
