These procedures are often chosen for patients with limited liver function reserve or tumors positioned where removing surrounding healthy tissue would be too risky, and they can be performed percutaneously, laparoscopically, or during open surgery depending on tumor location and size.

Common Bile Duct

The common bile duct is formed by the union of the cystic and common hepatic ducts and carries bile into the duodenum, often after passing through pancreatic tissue near its termination. Destruction of tissue within this duct is generally performed endoscopically, using laser, electrocautery, or photodynamic therapy to eliminate abnormal mucosa, small tumors, or dysplastic segments without removing the duct wall. The duct's course through the pancreatic head means that destructive procedures near its distal end require attention to the surrounding pancreatic parenchyma and the nearby ampullary opening. Because choledochal disease can recur or progress, documentation should indicate the specific duct segment treated and the energy source used, distinguishing this ablative approach from duct excision, repair, or bypass.

Approach: Via Natural or Artificial Opening

Via Natural or Artificial Opening means the instrument reaches the target site by passing through an existing body orifice, such as the mouth or urethra, or a surgically created opening like a stoma, without additional incision or scope guidance. It is distinguished from the Endoscopic variant by the absence of a visualizing scope, and from External by actually traversing into the body through that opening.
