Procedure Overview
Destruction procedures in the respiratory system eliminate abnormal or diseased tissue in place, using heat, cold, laser energy, or chemical agents, without physically removing the destroyed material from the body. A common application is ablation of an endobronchial tumor obstructing an airway, where a laser or cryoprobe delivered through a bronchoscope eradicates tissue in place, allowing the airway to reopen as the body clears the destroyed cells over time.
These procedures are chosen when a growth or lesion needs to be eliminated but cutting it out and removing it, as in an Excision, either isn't necessary or isn't feasible given its location or the patient's overall condition. Destruction is frequently used for palliative relief of airway obstruction from cancer or for treating certain benign lesions within the lung or bronchial tree.

Diaphragm
The diaphragm is the dome-shaped musculotendinous partition separating the thoracic and abdominal cavities, and destruction of this structure is an uncommon procedure, generally reserved for ablating an abnormal focus of tissue such as a small tumor deposit, an endometriotic implant, or an aberrant electrical pathway without physically removing diaphragmatic tissue. Energy-based methods, including cautery, laser, or radiofrequency, are applied to devitalize the target area in place, leaving the bulk of the muscle intact to preserve respiratory function. Because the diaphragm has right and left hemidiaphragms with distinct innervation and because injury here can compromise ventilation, the operative report should specify laterality and the exact location treated so the correct body part value is selected.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

To support a Destruction code, documentation must show that no tissue was excised or extracted, only obliterated through an energy source or agent such as laser, electrocautery, cryotherapy, or a caustic substance. Bronchoscopy reports describing "ablation," "fulguration," or "cryodestruction" of a lesion typically qualify. The most common coding error is applying Destruction when tissue was actually resected and sent to pathology, which belongs under Excision or Resection instead. Coders should also watch for cases combining destruction with a biopsy, which requires separate codes for each distinct action performed.
