Destruction procedures on bursae and ligaments eliminate abnormal or diseased tissue using direct energy or a destructive agent, such as electrocautery, laser, chemical injection, or extreme cold, rather than by cutting the tissue out. The targeted tissue is eradicated in place and remains in the body as it breaks down naturally afterward.

This approach is used for conditions like chronically inflamed bursal tissue that has not responded to conservative treatment, or for ablating a portion of ligament tissue causing persistent pain, such as in certain nerve or tissue ablation procedures aimed at symptom relief. It is often chosen when a less invasive option is preferred over open removal of tissue.

Recovery is generally shorter than for excisional surgery since no tissue is physically extracted, though patients may experience localized inflammation as the destroyed tissue resolves over the following weeks.

Left foot bursa and ligament includes the same plantar, intermetatarsal, and interphalangeal ligamentous structures and submetatarsal bursae as the right foot, commonly involved in chronic bursitis from ill-fitting footwear, bunion-related friction, or repetitive impact in runners. Destruction is used when a persistent bursal sac beneath a metatarsal head or a degenerated segment of plantar ligament needs to be obliterated in place, usually through minimally invasive percutaneous ablation given the limited soft tissue coverage over the plantar surface. Because the foot bears cumulative mechanical load with every step, clinicians document laterality carefully, as left foot pathology may reflect a distinct biomechanical pattern, such as asymmetric pronation, from the right, informing orthotic or footwear recommendations after treatment.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
