Procedure Overview

Destruction procedures in the peripheral nervous system eliminate all or part of a nerve using an energy source, chemical agent, or other destructive method, without physically cutting the tissue out of the body. Radiofrequency ablation of a peripheral nerve, chemical neurolysis with phenol or alcohol, and cryoablation of a painful nerve are common examples in this family.

These procedures are performed most often for chronic pain conditions, such as intractable neuropathic pain, complex regional pain syndrome, or pain from nerve entrapment, when more conservative treatments have failed. The intent is to interrupt the nerve's ability to transmit pain signals by damaging the nerve tissue itself, which can provide relief lasting months or longer depending on the technique used.

Head and Neck Sympathetic Nerve

The head and neck sympathetic nerves comprise the superior and middle cervical ganglia and the stellate ganglion, chains that regulate vasomotor tone, sweating, and pupillary function across the face, upper extremity, and neck. Destruction, commonly performed as a stellate ganglion neurolysis, is used to treat complex regional pain syndrome of the upper limb, refractory facial or upper extremity vascular pain, hyperhidrosis, and occasionally intractable cardiac arrhythmias mediated by sympathetic overactivity. The ganglion sits anterior to the C6 or C7 transverse process near the carotid sheath, so procedures are done under fluoroscopic or ultrasound guidance using chemical neurolytic agents or radiofrequency to minimize injury to adjacent vascular and airway structures. A transient Horner syndrome is an expected consequence of a technically successful procedure.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
