Procedure Overview

Destruction procedures in the hepatobiliary and pancreatic system eliminate abnormal or diseased tissue in place using heat, cold, chemicals, or another destructive agent, rather than cutting it out. This is commonly used for liver tumors that are not good candidates for surgical resection, where techniques such as radiofrequency ablation, microwave ablation, or cryoablation apply focused energy to kill the tissue while leaving it in the body to be reabsorbed over time.

These procedures are often chosen for patients with limited liver function reserve or tumors positioned where removing surrounding healthy tissue would be too risky, and they can be performed percutaneously, laparoscopically, or during open surgery depending on tumor location and size.

Liver, Left Lobe

The left lobe of the liver is smaller than the right and lies closer to the stomach and the anterior abdominal wall, making it more accessible for percutaneous ablative procedures in some patients. Destruction of a lesion isolated to the left lobe is coded separately from a right lobe or whole-liver procedure because it reflects a distinct anatomic target with its own vascular supply and margins. The left lobe's proximity to the stomach requires attention during ablation to avoid thermal injury to the gastric wall, and its position can make it more amenable to laparoscopic or open surgical ablation approaches when percutaneous access is limited by lesion location.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Qualifier: Irreversible Electroporation

This qualifier denotes irreversible electroporation, a nonthermal ablation technique that uses high-voltage electrical pulses to disrupt cell membranes and destroy targeted tissue. It differs from thermal-based qualifiers like ultrasonic or radiofrequency energy by sparing surrounding connective structures, which is why it is favored near sensitive anatomy such as vessels or ducts.
