Procedure Overview

Destruction procedures in the respiratory system eliminate abnormal or diseased tissue in place, using heat, cold, laser energy, or chemical agents, without physically removing the destroyed material from the body. A common application is ablation of an endobronchial tumor obstructing an airway, where a laser or cryoprobe delivered through a bronchoscope eradicates tissue in place, allowing the airway to reopen as the body clears the destroyed cells over time.

These procedures are chosen when a growth or lesion needs to be eliminated but cutting it out and removing it, as in an Excision, either isn't necessary or isn't feasible given its location or the patient's overall condition. Destruction is frequently used for palliative relief of airway obstruction from cancer or for treating certain benign lesions within the lung or bronchial tree.

Lung, Right

The right lung as a whole comprises three lobes and is the larger of the two lungs, and destruction coded at this level reflects a procedure targeting tissue across the organ broadly rather than a single lobe or segment. Such a procedure eradicates diffuse or multifocal abnormal tissue, for example widespread bullous disease or diffuse infectious or neoplastic involvement, using ablative energy delivered without physically removing lung tissue. This whole-organ level of destruction is less common than lobe-specific procedures and is typically reserved for disease that is not confined to a single anatomic subdivision. Given the right lung's greater volume and three-lobe structure, documentation at this level should make clear that the treated tissue spans beyond any single lobe to justify coding at the whole-lung level.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Qualifier: Laser Interstitial Thermal Therapy

Laser Interstitial Thermal Therapy is a qualifier used with the root operation Destruction, most often on brain tissue, denoting that tissue is ablated using a stereotactically placed laser probe delivering thermal energy under real-time MRI thermography guidance. It is distinguished from Stereoelectroencephalographic Radiofrequency Ablation, which uses electrode-delivered RF current instead of light energy, and from unqualified Destruction, which does not specify laser technique.

Coding & Documentation

To support a Destruction code, documentation must show that no tissue was excised or extracted, only obliterated through an energy source or agent such as laser, electrocautery, cryotherapy, or a caustic substance. Bronchoscopy reports describing "ablation," "fulguration," or "cryodestruction" of a lesion typically qualify. The most common coding error is applying Destruction when tissue was actually resected and sent to pathology, which belongs under Excision or Resection instead. Coders should also watch for cases combining destruction with a biopsy, which requires separate codes for each distinct action performed.

Commonly Confused With

ExcisionDestruction is frequently confused with Excision, since both can be performed with similar instruments, but Excision cuts out a portion of tissue and removes it from the body while Destruction eliminates it in place with no specimen retrieved.
