Procedure Overview

Destruction procedures in the respiratory system eliminate abnormal or diseased tissue in place, using heat, cold, laser energy, or chemical agents, without physically removing the destroyed material from the body. A common application is ablation of an endobronchial tumor obstructing an airway, where a laser or cryoprobe delivered through a bronchoscope eradicates tissue in place, allowing the airway to reopen as the body clears the destroyed cells over time.

These procedures are chosen when a growth or lesion needs to be eliminated but cutting it out and removing it, as in an Excision, either isn't necessary or isn't feasible given its location or the patient's overall condition. Destruction is frequently used for palliative relief of airway obstruction from cancer or for treating certain benign lesions within the lung or bronchial tree.

Lungs, Bilateral

Coding destruction to both lungs bilaterally reflects a procedure, or a combination of procedures during a single operative episode, in which abnormal tissue is eradicated in both the right and left lungs rather than confined to one side. This bilateral designation is used for diffuse processes affecting both lungs, such as widespread bullous disease or symmetric infectious or neoplastic involvement, where ablative energy is applied to eliminate diseased tissue in place on both sides without removing either lung. Because the two lungs differ in lobar structure, three lobes on the right and two on the left, bilateral destruction still requires attention to each lung's distinct anatomy during the procedure. Documentation should confirm that both lungs were addressed to justify the bilateral body part value rather than coding each side separately.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

ExcisionDestruction is frequently confused with Excision, since both can be performed with similar instruments, but Excision cuts out a portion of tissue and removes it from the body while Destruction eliminates it in place with no specimen retrieved.
