Procedure Overview

Destruction procedures in the upper joints eradicate abnormal or diseased tissue in place - using heat, cold, chemicals, laser, or another destructive energy source - without physically cutting the tissue out of the body. Nothing is removed for pathology examination or extracted; the tissue is destroyed where it lies.

This approach is used for conditions like a synovial lesion, a small benign growth, or abnormal tissue within a shoulder, elbow, wrist, or finger joint that can be eliminated with targeted energy rather than open excision. It is often chosen when the abnormal tissue is diffuse, difficult to isolate surgically, or when a less invasive energy-based approach can achieve the same clinical goal as cutting it away.

Thoracic Vertebral Disc

Thoracic vertebral discs sit between the twelve thoracic vertebral bodies and are inherently less mobile than their cervical or lumbar counterparts because the rib cage restricts thoracic spine motion, which also makes symptomatic thoracic disc disease comparatively uncommon. When a thoracic disc does become a pain generator, often from age-related degeneration, destructive treatment such as radiofrequency or chemical ablation targets the disc's nerve supply to relieve pain without excising the disc material. The thoracic spinal canal is narrower relative to the cord diameter than elsewhere in the spine, so image-guided precision is particularly important to avoid neural injury. Documentation must specify the exact thoracic level involved, since each disc space is coded individually and confusion with adjacent cervicothoracic or thoracolumbar discs should be avoided.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
