Procedure Overview
Destruction procedures in the urinary system eliminate abnormal tissue, such as a bladder tumor, ureteral lesion, or urethral growth, by applying energy or a caustic agent directly to it rather than cutting it out. Fulguration of bladder tumors through a cystoscope is the most familiar example, where electrical current chars the abnormal tissue in place.
This approach is chosen when a lesion is small, superficial, or diffuse enough that outright excision would be unnecessarily invasive, or when preserving the surrounding structure matters more than retrieving a specimen. It is common in the ongoing management of recurrent low-grade bladder tumors, where repeated surveillance and destruction cycles are standard care.

Ureter, Left
The left ureter descends from the left renal pelvis toward the bladder, passing near the sigmoid colon, left ovarian or spermatic vessels, and the pelvic sidewall. When abnormal tissue such as a urothelial tumor or endometrial implant is found along this course, destruction eradicates it in place using an energy modality delivered ureteroscopically, most often laser or electrocautery, without taking out any part of the ureter. This approach preserves ureteral continuity and avoids the stricture risk associated with segmental excision, which matters given the left ureter's proximity to the sigmoid mesentery where scarring can compound existing pathology. Records should specify that the lesion was ablated rather than cut out, since even partial-thickness resection during the same encounter would shift the procedure to a different root operation.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
