Procedure Overview
Destruction procedures in the urinary system eliminate abnormal tissue, such as a bladder tumor, ureteral lesion, or urethral growth, by applying energy or a caustic agent directly to it rather than cutting it out. Fulguration of bladder tumors through a cystoscope is the most familiar example, where electrical current chars the abnormal tissue in place.
This approach is chosen when a lesion is small, superficial, or diffuse enough that outright excision would be unnecessarily invasive, or when preserving the surrounding structure matters more than retrieving a specimen. It is common in the ongoing management of recurrent low-grade bladder tumors, where repeated surveillance and destruction cycles are standard care.

Urethra
The urethra is the conduit carrying urine from the bladder to the external meatus, and its length and surrounding structures differ substantially between male and female anatomy, with the male urethra traversing the prostate and penile shaft. Destruction of urethral tissue is performed for condylomata, urethral caruncle in females, or small papillary lesions, using fulguration, laser, or chemical cautery applied endoscopically or topically to eradicate the abnormal tissue in place. Because the urethral lumen is narrow and lined by tissue prone to scarring, energy application must be limited to avoid inducing a stricture, a known complication of aggressive ablation along this canal. Documentation should specify the destructive modality used and confirm that no urethral tissue was physically excised, since excision of a caruncle or lesion would instead be coded as a different root operation.
Approach: Percutaneous Endoscopic
Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
