Procedure Overview

Dilation procedures widen a lower vein that has become narrowed, most commonly the iliac vein or another deep vein in the pelvis or leg affected by scarring from a prior blood clot or external compression. The left renal vein is longer than the right and crosses the midline anterior to the aorta, often coursing between the aorta and superior mesenteric artery, a relationship that predisposes it to extrinsic compression known as nutcracker syndrome. Dilation of the left renal vein is performed to relieve stenosis from this anatomic compression, from thrombosis, or from post-transplant narrowing, with the goal of reducing renal venous hypertension, hematuria, or flank pain associated with impaired outflow. The intervention is generally performed percutaneously with balloon angioplasty, and stenting is frequently considered given the mechanical nature of the compression in nutcracker-type cases. Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
