Tendon division procedures cut through a tendon to separate it, without draining any fluid or removing tissue. The classic example is a tenotomy performed to release excessive tension, most often in children or adults with contractures from conditions like clubfoot, cerebral palsy, or spasticity, where lengthening a tight tendon improves joint mobility and gait. The tendon is deliberately transected, sometimes at multiple levels, and allowed to heal in a lengthened position, or it may be left divided to eliminate an abnormal pulling force altogether.

Because the intent is purely to cut and separate rather than to remove tissue or drain fluid, this is a comparatively contained procedure that can often be done percutaneously with a small blade, avoiding an open incision. Patients typically need bracing or casting afterward to hold the limb in the corrected position while the tendon reattaches or scars into its new length.

On the left side, the ankle tendon most commonly referenced for division is the Achilles tendon, along with the tibialis, peroneal, and flexor or extensor tendons that pass through the ankle region. Achilles tenotomy remains a standard step in the correction of equinus contracture and clubfoot deformity, performed when passive stretching fails to achieve adequate dorsiflexion, and the procedure is typically followed by immobilization to allow the tendon to heal at a functional length. Other ankle tendons may be divided to relieve a localized contracture or spastic pull affecting foot position. Given the number of tendons converging at the ankle, precise identification of the one divided is necessary, as each governs a different component of foot motion.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

A Division code applies when the documentation describes cutting or transecting the tendon to separate it, with no fluid or gas drainage and no tissue taken out of the body. A common coding pitfall is confusing tenotomy performed for lengthening with tenotomy performed as an approach step to reach a deeper structure, which would not be separately coded. Coders also sometimes miss that if the surgeon removes a piece of the tendon rather than simply cutting through it, the case belongs under Excision instead, so precise reading of whether tissue was taken out is essential.

Commonly Confused With

ReleaseRelease is the family most often mixed up with Division, since both can loosen a tight tendon, but Release frees the tendon from surrounding restrictive tissue without cutting through the tendon itself, while Division cuts through the tendon's own substance.
ExcisionExcision can also resemble Division when a tenotomy note mentions removing a small tendon fragment, but true Division involves no tissue leaving the body.
