Tendon division procedures cut through a tendon to separate it, without draining any fluid or removing tissue. The classic example is a tenotomy performed to release excessive tension, most often in children or adults with contractures from conditions like clubfoot, cerebral palsy, or spasticity, where lengthening a tight tendon improves joint mobility and gait. The tendon is deliberately transected, sometimes at multiple levels, and allowed to heal in a lengthened position, or it may be left divided to eliminate an abnormal pulling force altogether.

Because the intent is purely to cut and separate rather than to remove tissue or drain fluid, this is a comparatively contained procedure that can often be done percutaneously with a small blade, avoiding an open incision. Patients typically need bracing or casting afterward to hold the limb in the corrected position while the tendon reattaches or scars into its new length.

Hip Tendon, Left

The left hip tendon designation covers the tendinous structures crossing the hip joint on that side, most notably the iliopsoas and adductor tendons. Surgeons divide these tendons to correct spastic contracture in neuromuscular disorders, to relieve painful snapping hip syndrome, or to address a fixed flexion or adduction deformity that limits ambulation. The iliopsoas tendon in particular passes close to the femoral neurovascular bundle, so its division requires careful exposure to avoid injury to nearby structures. Because a tenotomy at the hip can significantly alter gait mechanics, the specific tendon and the extent of the cut are typically documented in detail, distinguishing a complete release from a partial lengthening procedure that might otherwise be coded differently.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

A common coding pitfall is confusing tenotomy performed for lengthening with tenotomy performed as an approach step to reach a deeper structure, which would not be separately coded. Commonly Confused With

ReleaseRelease is the family most often mixed up with Division, since both can loosen a tight tendon, but Release frees the tendon from surrounding restrictive tissue without cutting through the tendon itself, while Division cuts through the tendon's own substance.
