Cutting into a body part, without draining fluids and/or gases from the body part, in order to separate or transect a body part
Procedure Overview
Division procedures in this family involve cutting into a bursa or ligament to separate or transect it, without removing any tissue and without draining fluid. The goal is to release tension, relieve compression, or free a structure that has become tight, thickened, or scarred. A familiar example is release of the transverse carpal ligament to relieve carpal tunnel syndrome, or sectioning a tight ligament that is restricting joint motion after injury or contracture.
Because nothing is removed, the ligament or bursa remains in the body, just cut through so its two ends separate or its restrictive effect is eliminated. Surgeons choose this approach when the structure itself is not diseased but is mechanically causing a problem, such as nerve entrapment or joint stiffness.
Recovery generally focuses on restoring motion and function once the compressive or restrictive tissue has been released, and many division procedures are done through small incisions or endoscopically.

Rib-associated bursae and ligaments include the costotransverse and costovertebral ligaments anchoring each rib to the corresponding vertebra, along with any local bursae reducing friction where the rib articulates posteriorly. Division of a costotransverse or costovertebral ligament may be performed to mobilize a rib during thoracic surgical exposure, to release a structure contributing to restricted rib excursion, or as part of a procedure addressing a rib deformity affecting the thoracic cage. Because each rib's stability at the vertebral articulation depends on these ligaments, surgeons divide them selectively and typically only to the extent needed to achieve mobility or exposure, since excessive division can affect the mechanical integrity of the rib cage's connection to the spine and its role in respiratory motion.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
