Procedure Overview

Drainage procedures on the muscles remove fluid or gas that has accumulated within or around a muscle, most often pus from an abscess, blood from a hematoma, or infected fluid tracking along a muscle compartment. An incision is made into the muscle to allow the material to escape, sometimes with a drain left in place afterward to keep the area open while it continues to resolve.

These procedures are usually urgent, addressing infection or swelling that is causing pain, pressure, or risk of spreading, such as a psoas abscess or a hematoma large enough to compress surrounding structures. The muscle tissue itself is not removed; only the fluid or gas collection is evacuated.

Because the underlying cause, such as infection, often needs treatment beyond the procedure itself, drainage is frequently paired with antibiotics or other management addressing the source of the collection.

The left abdomen muscle group refers to the rectus abdominis, oblique, and transversus abdominis muscles on the left side of the abdominal wall, a site prone to rectus sheath hematoma in anticoagulated patients or after strain, as well as abscess formation following surgical incisions such as ostomy sites or prior laparotomy. Because the abdominal wall musculature lies just superficial to the peritoneal cavity, careful assessment is needed to confirm a collection has not extended into the intraperitoneal space, which would change both management and coding. Ultrasound or CT typically confirms the collection's plane before drainage is attempted. The left designation is used when the process is confined to that side, which is common given the frequently unilateral nature of rectus sheath bleeding and wall abscesses tied to a specific incision or injection site.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
