Procedure Overview

A needle aspiration of a swollen knee after an injury, or surgical drainage of a septic hip in an infant, both fall under this family. The purpose is either diagnostic - sending the fluid for lab analysis to identify infection or crystal disease - or therapeutic, relieving pressure and pain caused by the buildup.

These procedures range from a simple bedside or office needle aspiration to an incision-and-drainage operation performed in the operating room when infection is suspected and thorough irrigation is needed. Septic arthritis in particular is treated urgently this way, since untreated joint infection can rapidly destroy cartilage.

Patients usually have this done for sudden joint swelling, warmth, or pain, and the procedure itself is often quick, though washout of an infected joint may involve repeat visits to the operating room.

Ankle Joint, Right

The right ankle joint is the hinge articulation formed primarily by the tibia, fibula, and talus, encased in a relatively tight capsule that can distend with fluid following trauma, gout, or infection introduced hematogenously or from adjacent cellulitis. Because the joint space is narrower than that of the knee or hip, drainage is often performed with ultrasound or fluoroscopic guidance to ensure accurate needle placement into the anterior joint recess while avoiding the extensor tendons and neurovascular structures crossing the ankle. Septic arthritis of the ankle is treated as urgent given the joint's limited soft tissue reserve and proximity to weight-bearing structures. The right-sided designation should be verified against imaging, especially in patients with a history of bilateral ankle problems or prior surgery.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
