Drainage procedures in the respiratory system remove fluid, air, blood, or pus that has built up somewhere it doesn't belong, most often the pleural space between the lung and chest wall, but also within the lung tissue, bronchi, or trachea. A collapsed lung from trapped air (pneumothorax), an infected fluid collection (empyema), a malignant pleural effusion, or a lung abscess are all situations where drainage restores normal breathing mechanics and relieves pressure that would otherwise compress the lung.

Carina
The carina, the cartilaginous ridge marking the tracheal bifurcation into the right and left main bronchi, is an uncommon but clinically significant site for drainage when secretions, blood, or purulent debris pool at this junction and threaten to obstruct both lungs at once. Bronchoscopic suctioning or lavage directed specifically at the carinal region clears this material under direct visualization, distinct from suctioning performed more diffusely throughout the tracheobronchial tree or confined to a single main bronchus. Because obstruction at the carina can compromise ventilation bilaterally, this drainage is documented separately from procedures on the trachea or an individual main bronchus, with the operative note specifying that the accumulation and its removal were localized to the carinal ridge itself.

Via Natural or Artificial Opening means the instrument reaches the target site by passing through an existing body orifice, such as the mouth or urethra, or a surgically created opening like a stoma, without additional incision or scope guidance.
