The left external iliac artery extends from the common iliac artery to the inguinal ligament, where it becomes the femoral artery, and shares the right side's vulnerability to access-site hematoma after catheterization, stenting, or bypass surgery. Drainage addresses a periarterial or retroperitoneal collection that forms along its relatively superficial course adjacent to the psoas muscle and iliac lymph nodes. Because a large hematoma in this location can compress the femoral nerve or extend into the retroperitoneum, imaging is used to characterize the collection's size and extent before deciding on approach. Percutaneous, image-guided catheter drainage is preferred for a localized collection, while open surgical drainage is reserved for larger or infected hematomas, particularly those involving a vascular graft or stent.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
