Drainage procedures in the urinary system remove fluid, pus, or urine that is trapped or backed up because of a blockage, infection, or injury. The most familiar examples are placement of a nephrostomy tube through the skin into the kidney, a suprapubic catheter into the bladder, or drainage of an abscess around the kidney or bladder. These procedures relieve pressure, prevent kidney damage from obstructed urine flow, and give infected fluid a way out of the body while the underlying problem is treated.

Most patients encounter drainage as an urgent, temporary measure rather than a definitive fix - a nephrostomy tube buys time until a stone or tumor causing a blocked ureter can be addressed, for instance. The tube or catheter is usually left in place and monitored, then removed once imaging or lab work shows the fluid has cleared and the underlying cause is resolved.

The renal pelvis is the funnel-shaped chamber where urine collects before passing into the ureter, and on the right side it is a common target for percutaneous drainage when obstruction causes pyonephrosis or when a stone is lodged at the ureteropelvic junction. Direct pelvis access, as opposed to broader renal parenchymal drainage, allows more targeted decompression and is often the first step before definitive stone removal or stricture repair. Antegrade pyelography performed through the same access can help delineate the anatomy. Coders should confirm from the operative note whether the catheter tip terminates specifically within the pelvis rather than the calyces or renal substance, since precise body part selection depends on that detail.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
