Procedure Overview

Drainage procedures on the lower bones remove fluid or gas that has built up within or around a bone, most often pus from an infection such as osteomyelitis or a subperiosteal abscess. A surgeon opens a channel into the affected area of the pelvis, femur, tibia, or another lower bone and lets the collection escape, sometimes leaving a drain in place to allow continued outflow.

This family of procedures is used when infection, blood, or another fluid collection is causing pain, swelling, or systemic illness and needs to be evacuated to relieve pressure and allow antibiotics or the body's own defenses to bring the infection under control.

Lower Femur, Right

The lower right femur consists of the widened metaphysis and the medial and femoral condyles just above the knee joint, an area rich in cancellous bone and close to the growth plate in younger patients. Drainage here commonly addresses osteomyelitis that has spread from the metaphysis, periprosthetic infection following total knee arthroplasty, or an abscess adjacent to the distal growth plate that risks growth disturbance if untreated. The proximity to the knee joint capsule means infection can extend intra-articularly, so surgeons must determine whether the collection is confined to bone or has breached into the joint space. Because this region is relatively superficial anteriorly and medially, drainage can often be performed through a limited incision with direct visualization.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Qualifier: Diagnostic

Applied chiefly to Excision, this qualifier marks that tissue was removed specifically for diagnostic purposes, i.e., a biopsy intended for pathologic evaluation rather than to treat disease. It distinguishes biopsy-type excisions from therapeutic excisions of the same body part, which are coded without this qualifier even though the physical technique may be identical.

Coding & Documentation

The documentation should describe fluid or gas being let out of the bone and specify whether a drainage device, such as a catheter or drain, was left behind, since that determines whether a qualifier for drainage device applies. The exact bone and the depth of approach (percutaneous versus open) should be clearly stated to select the right approach value.

A common error is coding Drainage when the procedure actually removed necrotic bone tissue along with the fluid - if dead or infected bone was debrided and taken out, that portion of the case is better captured under Excision, and both root operations may need to be coded if both were performed. Coders should also avoid defaulting to Drainage for procedures where the bone itself was cut without any fluid evacuation.

ExcisionExcision (0QB) is commonly performed alongside Drainage in infected bone, and the two are distinguished by whether tissue was cut out and removed (Excision) versus fluid being let out (Drainage) - both codes may apply to the same operative episode.
