Drainage procedures in the respiratory system remove fluid, air, blood, or pus that has built up somewhere it doesn't belong, most often the pleural space between the lung and chest wall, but also within the lung tissue, bronchi, or trachea. A collapsed lung from trapped air (pneumothorax), an infected fluid collection (empyema), a malignant pleural effusion, or a lung abscess are all situations where drainage restores normal breathing mechanics and relieves pressure that would otherwise compress the lung.

Lower Lung Lobe, Left
The lower lobe of the left lung sits in the posterior and inferior left chest and, like its right-sided counterpart, is the most gravity-dependent portion of the lung, making it prone to accumulating parapneumonic effusion, empyema, and abscess material in patients with prolonged recumbency or aspiration risk. Drainage at this parenchymal level addresses fluid collections within the lung tissue rather than secretions confined to the bronchial lumen. Its position adjacent to the diaphragm, spleen, and descending aorta means image-guided drain placement requires careful trajectory planning to avoid these structures, particularly the spleen given its proximity on the left side unlike the liver on the right. Necrotizing pneumonia and basal segment abscesses commonly localize here. Documentation should identify the lower lobe specifically as the parenchymal site of drainage.
Approach: Percutaneous Endoscopic
Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
