Procedure Overview
Drainage procedures on the lower bones remove fluid or gas that has built up within or around a bone, most often pus from an infection such as osteomyelitis or a subperiosteal abscess. A surgeon opens a channel into the affected area of the pelvis, femur, tibia, or another lower bone and lets the collection escape, sometimes leaving a drain in place to allow continued outflow.
This family of procedures is used when infection, blood, or another fluid collection is causing pain, swelling, or systemic illness and needs to be evacuated to relieve pressure and allow antibiotics or the body's own defenses to bring the infection under control.

Patella, Right
The right patella is the sesamoid bone embedded within the quadriceps tendon, sitting directly under the skin at the front of the knee with minimal soft tissue coverage. Drainage of the patella itself is uncommon but may be required for osteomyelitis following open fracture, surgical fixation, or direct penetrating injury, since the bone's superficial position makes it susceptible to contamination from overlying skin infection or wound breakdown. Distinguishing a true intraosseous or subperiosteal patellar collection from the far more common prepatellar bursitis is clinically important, as the latter involves the bursa rather than bone. Given the thin soft tissue envelope, even small collections can be palpable, and drainage is often achieved through a limited direct incision over the affected area.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
Device: Drainage Device
Drainage Device denotes a device such as a tube or catheter left in place to remove fluid, blood, or air from a body part or cavity following a procedure. It is distinguished from Monitoring Device, which senses and records physiologic data rather than evacuating substances from the body.
Coding & Documentation
The documentation should describe fluid or gas being let out of the bone and specify whether a drainage device, such as a catheter or drain, was left behind, since that determines whether a qualifier for drainage device applies. The exact bone and the depth of approach (percutaneous versus open) should be clearly stated to select the right approach value.
A common error is coding Drainage when the procedure actually removed necrotic bone tissue along with the fluid - if dead or infected bone was debrided and taken out, that portion of the case is better captured under Excision, and both root operations may need to be coded if both were performed. Coders should also avoid defaulting to Drainage for procedures where the bone itself was cut without any fluid evacuation.
