Pericardial Cavity

Drainage of the pericardial cavity removes fluid, blood, or purulent material from the potential space between the visceral and parietal layers of the pericardium surrounding the heart, most urgently performed for cardiac tamponade but also for malignant or uremic effusions and purulent pericarditis. Because even a modest volume of fluid accumulating quickly in this relatively noncompliant sac can impair diastolic filling and cause hemodynamic collapse, pericardiocentesis is often emergent and image-guided, while surgical drainage through a subxiphoid window or thoracotomy is chosen for recurrent or loculated effusions. The proximity of the great vessels and coronary arteries makes needle or catheter placement technically demanding, and documentation should specify whether the approach was percutaneous or open, since this materially affects the approach value assigned.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
