Procedure Overview

The surgeon opens or punctures the area next to the nerve and lets the fluid escape, either through a needle and syringe or an incision, sometimes leaving a temporary tube in place so drainage continues over several days. Because peripheral nerves have little room to swell before they are damaged, timing often matters more here than in many other body systems.

Pudendal Nerve

The pudendal nerve travels through the pelvis near the ischial spine and Alcock's canal, an anatomically narrow passage where fluid collections from pelvic surgery, childbirth trauma, or perineal abscess can cause significant nerve compression and pain. Drainage of a collection along this nerve addresses symptoms such as perineal pain or bowel and bladder dysfunction that result from the fluid pressing on the nerve within its confined osteofibrous tunnel. Because the pudendal nerve's course is deep and adjacent to the rectum and vaginal or perineal structures, image guidance is frequently used to localize the collection accurately and minimize risk to surrounding tissue. Documentation should reflect the precise perineal or pelvic subsite, since this nerve's segmental anatomy along Alcock's canal affects surgical planning.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

A common error is coding a nerve decompression or neurolysis as Drainage when no fluid was actually removed; if the surgeon only frees scar tissue around the nerve, that is Release, not Drainage.
