Procedure Overview

Drainage procedures on the lower bones remove fluid or gas that has built up within or around a bone, most often pus from an infection such as osteomyelitis or a subperiosteal abscess. A surgeon opens a channel into the affected area of the pelvis, femur, tibia, or another lower bone and lets the collection escape, sometimes leaving a drain in place to allow continued outflow.

This family of procedures is used when infection, blood, or another fluid collection is causing pain, swelling, or systemic illness and needs to be evacuated to relieve pressure and allow antibiotics or the body's own defenses to bring the infection under control.

Sacrum

The sacrum is a triangular fused bone closing the posterior pelvic ring, sitting just beneath the skin overlying the buttocks and lower back. Its subcutaneous position and proximity to the anus and perineum make it prone to abscesses arising from pressure injuries, pilonidal disease, or presacral infection that erodes into bone, producing sacral osteomyelitis. Drainage addresses these collections, which often track along fascial planes and can be deep and irregular rather than a simple pocket. Because the sacral canal carries the lower sacral nerve roots, care is taken to avoid neural structures during instrumentation, and chronic wounds in this region are frequently complicated by poor local blood supply and contamination from adjacent skin and bowel flora, which affects healing and follow-up management.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Qualifier: Diagnostic

Applied chiefly to Excision, this qualifier marks that tissue was removed specifically for diagnostic purposes, i.e., a biopsy intended for pathologic evaluation rather than to treat disease. It distinguishes biopsy-type excisions from therapeutic excisions of the same body part, which are coded without this qualifier even though the physical technique may be identical.

Coding & Documentation

The documentation should describe fluid or gas being let out of the bone and specify whether a drainage device, such as a catheter or drain, was left behind, since that determines whether a qualifier for drainage device applies. The exact bone and the depth of approach (percutaneous versus open) should be clearly stated to select the right approach value.

A common error is coding Drainage when the procedure actually removed necrotic bone tissue along with the fluid - if dead or infected bone was debrided and taken out, that portion of the case is better captured under Excision, and both root operations may need to be coded if both were performed. Coders should also avoid defaulting to Drainage for procedures where the bone itself was cut without any fluid evacuation.
