Procedure Overview
Drainage procedures remove fluid, pus, blood, or air that has accumulated abnormally in a mouth or throat structure, most commonly to treat an abscess, hematoma, or infected space that is causing pain, swelling, or airway compromise. A common example is incision and drainage of a peritonsillar abscess, where a collection of pus beside the tonsil is opened and evacuated to relieve pressure and clear the infection.
This procedure is usually performed urgently when a fluid collection is causing significant symptoms or risking further spread of infection, and it can be done with a simple needle aspiration or a small incision depending on the size and location of the collection.

The left submaxillary gland, situated in the submandibular triangle and draining saliva through Wharton's duct, is frequently affected by sialolithiasis, which can obstruct outflow and predispose the gland to acute bacterial infection and abscess. Drainage addresses these infected or obstructed collections, releasing pressure and purulent material to control infection before it tracks along fascial planes of the neck. The operative approach, whether transoral near the duct orifice or through a skin incision over the gland, must account for the nearby marginal mandibular branch of the facial nerve, lingual nerve, and facial vessels. This root operation applies specifically to fluid evacuation from the left-sided gland; documentation should distinguish it from procedures on the contralateral gland or from any resection of glandular tissue, which would be classified differently.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

A frequent mistake is coding a drain placement that was left indwelling as though it were only a single Drainage encounter without appropriately capturing the device, or confusing an abscess drainage with an Excision when a wall of the cavity was actually removed. Precise identification of the anatomic site, such as peritonsillar space versus retropharyngeal space, is also necessary for accurate code assignment.
