Procedure Overview
Drainage procedures on the upper veins remove fluid, most often pooled blood or infected material, from veins in the arms, chest wall, neck, or shoulder region. Surgeons perform these when a vein has become blocked by a clot, has ruptured, or has developed an abscess or hematoma that needs to be evacuated to relieve pressure, pain, or the risk of infection spreading. A needle, catheter, or small incision is used to reach the vein and let the trapped fluid escape, sometimes with a drain left in place for a period afterward.

The left vertebral vein courses through the cervical transverse foramina in parallel with the vertebral artery on the opposite side, draining posterior fossa and cervical spinal venous blood into the left brachiocephalic vein. Its protected position within the bony canal makes direct access difficult, so drainage is usually undertaken in the context of cervical trauma, iatrogenic injury during spine instrumentation, or an infection tracking along the perivertebral space that has produced a collection compressing the vein. Evacuating such a collection relieves pressure within the confined canal and reduces the risk of extension toward the intracranial venous system or compromise of the adjacent nerve roots and spinal cord.
Approach: Percutaneous Endoscopic
Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
