Excision procedures on the upper arteries involve cutting out a portion of an artery in the chest, neck, shoulder, or arm without replacing the tissue removed. Surgeons perform this when a segment of vessel wall has been damaged by trauma, contains a diseased or aneurysmal section, or holds a tumor or growth that needs to be removed for diagnosis or treatment. Common targets include the subclavian, axillary, brachial, and carotid arteries, along with the thoracic aorta and its major branches supplying the upper body.

The removed tissue is often small relative to the vessel and does not always require reconstruction of the artery afterward, distinguishing it from more extensive resections. Patients undergo this procedure to address localized disease, obtain a biopsy sample, or clear away scarred or narrowed sections that are interfering with blood flow to the arms, neck, or brain.

Brachial Artery, Right

The right brachial artery is the direct continuation of the axillary artery running down the medial aspect of the upper arm, a vessel familiar to clinicians as the standard site for blood pressure auscultation and a common access point for catheter-based procedures. Excision of a segment is typically indicated for aneurysm, often related to repeated catheterization or dialysis access complications, for traumatic injury, or for a mass adjacent to the vessel wall. Its relatively superficial course alongside the median nerve makes it more accessible than the more proximal upper extremity arteries, but nerve proximity still requires careful dissection, and the operative note should specify whether the excised segment was replaced with graft material or simply removed.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

ExtirpationExtirpation is the family most often mixed up with Excision here, since both remove material from the artery, but Extirpation applies specifically to abnormal solid matter such as thrombus, embolus, or plaque rather than a portion of the vessel wall itself.
ResectionResection is similarly close but applies when an entire named arterial segment or branch is cut out rather than only part of it.
FragmentationFragmentation can also resemble Extirpation in intent but breaks obstructing material into pieces in place rather than removing it, so the operative note's description of whether material was taken out whole, in pieces, or left behind after being broken up determines which family applies.
