Procedure Overview

Excision in the endocrine system refers to cutting out a portion of a gland, such as removing a nodule from the thyroid, a section of the adrenal cortex, or one lobe of the thyroid, without replacing what is removed. Unlike removal of an entire organ, excision takes out only part of the structure, leaving the remainder in place to continue functioning.

This approach is used when a biopsy is needed to evaluate a suspicious nodule, when a benign growth is causing symptoms or cosmetic concern, or when a portion of a gland is producing hormones abnormally and needs to be reduced or removed while preserving the rest of the gland's function. Carotid Bodies, Bilateral

A bilateral carotid body excision addresses both chemoreceptor nodules at the left and right carotid bifurcations in the same operative episode, a less common scenario usually reserved for familial paraganglioma syndromes in which multiple carotid body tumors develop synchronously on both sides of the neck. Because bilateral resection carries meaningful risk of impairing baroreceptor and chemoreceptor reflexes that help regulate blood pressure and respiratory drive, surgeons often stage the two sides across separate operations rather than excise both simultaneously; when both are nonetheless done in one episode, the bilateral body part value applies. Each side still requires its own careful dissection off the carotid adventitia, and any known genetic predisposition, such as an SDHB mutation, may prompt closer surveillance for additional paraganglion sites.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Coders should also watch for excisional biopsies coded separately from a more extensive excision performed in the same session, and confirm the correct body part value is used since endocrine structures like the thyroid lobes and parathyroid glands each have distinct values.
