Procedure Overview

Excision procedures on the muscles remove a portion of a muscle, but not the entire structure, most commonly to obtain a biopsy sample, to debride damaged or infected tissue, or to remove a localized mass or lesion within the muscle. The remainder of the muscle stays in place and continues to function.

This family covers a wide range of clinical situations, from a small diagnostic biopsy of muscle tissue to evaluate a suspected myopathy, to debridement of necrotic muscle following trauma or severe infection, to removal of a benign tumor confined to a muscle. What ties these together is that only part of the muscle is taken, leaving the rest intact.

Because the tissue removed is often sent for pathological or microbiological analysis, excision plays a diagnostic role in many cases in addition to a therapeutic one.

Lower Arm and Wrist Muscle, Right

The lower arm and wrist muscles on the right side comprise the flexor and extensor groups of the forearm along with the small muscles crossing the wrist, structures packed closely together and interwoven with tendons that extend into the hand. Excision in this region is typically performed to remove a soft tissue mass, ganglion-associated muscle tissue, or scarred muscle following a laceration or repetitive strain injury, and the density of tendons, the median, ulnar, and radial nerves, and the radial and ulnar arteries in this compact space demands meticulous dissection to avoid collateral damage. Because forearm muscles are functionally paired for flexion and extension, surgeons take care to excise only the affected muscle or muscle segment, and precise documentation of which specific forearm or wrist muscle was involved is important given the many closely packed structures in this area.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

Qualifier: Diagnostic

Applied chiefly to Excision, this qualifier marks that tissue was removed specifically for diagnostic purposes, i.e., a biopsy intended for pathologic evaluation rather than to treat disease. It distinguishes biopsy-type excisions from therapeutic excisions of the same body part, which are coded without this qualifier even though the physical technique may be identical.

Coding & Documentation

Documentation should describe cutting out a portion, sample, or segment of muscle, with the specific muscle named and the extent of tissue removed described clearly enough to distinguish it from removal of the whole muscle. Pathology or culture results are often present when the excision was diagnostic.

One recurring mistake is coding Excision for debridement performed at the bedside without excisional technique, which if truly nonexcisional would not be captured under Muscles Excision at all, or coding Excision when in fact the entire muscle was taken, which should be Resection. Coders also need to be careful not to code a biopsy as Extraction, since cutting tissue out is Excision even when the intent is purely diagnostic.

Commonly Confused With

ResectionResection is the direct counterpart, separated only by extent: taking the entire named muscle is Resection, while taking any lesser portion is Excision, so operative documentation of the amount removed is essential.
DestructionDestruction is sometimes confused with Excision when tissue is both destroyed and partially removed in the same operative session; the excised portion is coded separately from any destroyed tissue that was not taken out of the body.
DrainageDrainage is distinct in that it removes fluid rather than solid muscle tissue, even though both may occur through the same incision during treatment of an abscess.
