Procedure Overview
Tendon excision procedures cut out and remove a portion of a tendon without replacing it, typically to address a tumor, a chronically degenerated segment, a painful nodule, or scar tissue that is limiting motion. A well-known example is excision of a trigger finger nodule or a portion of tendon affected by tenosynovitis that has not responded to more conservative treatment, as well as biopsy of a suspicious mass for diagnostic purposes.
Because only part of the tendon is taken and the remainder is left in place, the surgeon must weigh how much tissue can be safely removed without compromising the tendon's mechanical function. Excision is performed to remove degenerated or hypertrophic tendon tissue, commonly encountered in chronic Achilles tendinosis, peroneal tendon tears with retained scar, or posterior tibial tendon dysfunction contributing to acquired flatfoot. Surgeons often debride only the pathologic segment while preserving the functional portion of the tendon to maintain biomechanical integrity. Given the compartmentalized anatomy of the lower leg, with tendons running in distinct osteofascial tunnels near the malleoli, accurate documentation of laterality and specific tendon is necessary to differentiate this procedure from adjacent muscle or retinaculum excision.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
