Excision procedures remove a portion of a lymph node, the spleen, the thymus, or another lymphatic or hemic structure, without replacing it, most commonly to obtain a tissue sample for diagnosis. Lymph node biopsies fall squarely in this family and are frequently used to stage cancers such as lymphoma or breast cancer, or to investigate unexplained swelling.
Because these excisions are usually diagnostic, the tissue removed is sent to pathology for microscopic examination, and the results often guide decisions about further treatment such as chemotherapy or additional surgery. Lymphatic, Left Neck
The left cervical lymph node chain, situated along the carotid sheath and jugular vein, is notable for the left supraclavicular node, or Virchow's node, which receives drainage from abdominal and thoracic viscera via the thoracic duct and can be the first clinically apparent sign of a distant gastrointestinal or pelvic malignancy. Excision of a node from this chain is therefore often pursued specifically to investigate suspected metastatic disease from below the diaphragm, in addition to local causes such as thyroid or laryngeal cancer. The thoracic duct's termination near the left subclavian-jugular confluence means dissection here carries a distinct risk of chyle leak that is less of a concern on the right side. As with the contralateral chain, the vagus and spinal accessory nerves and the great vessels of the neck must be protected during exposure.
Approach: Percutaneous Endoscopic
Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
