Procedure Overview
Excision procedures remove a portion of a lymph node, the spleen, the thymus, or another lymphatic or hemic structure, without replacing it, most commonly to obtain a tissue sample for diagnosis. Lymph node biopsies fall squarely in this family and are frequently used to stage cancers such as lymphoma or breast cancer, or to investigate unexplained swelling.
Because these excisions are usually diagnostic, the tissue removed is sent to pathology for microscopic examination, and the results often guide decisions about further treatment such as chemotherapy or additional surgery. A partial splenectomy performed to preserve some immune function while removing damaged tissue is another example within this family.
These procedures range from a quick outpatient biopsy under local anesthesia to a more involved surgical excision requiring general anesthesia, depending on the location and size of the tissue being sampled.

Lymphatic, Right Axillary
The right axillary nodal basin, organized into levels I through III relative to the pectoralis minor muscle, is the principal drainage pathway for the right breast and upper limb and is most often excised, whether as a single sentinel node or a limited sampling, in the staging and treatment of right-sided breast cancer or melanoma of the arm or trunk. Dissection in this region requires identifying and preserving the long thoracic nerve supplying the serratus anterior, the thoracodorsal neurovascular bundle to the latissimus dorsi, and the axillary vein, since injury to any of these can cause a winged scapula, shoulder dysfunction, or lymphedema. The intercostobrachial nerve, which supplies sensation to the medial arm, is frequently encountered and sometimes sacrificed. Because axillary excision carries a recognized risk of chronic lymphedema, the extent of nodal removal is typically limited to what staging requires.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
