Because only a portion of the structure is taken and the joint itself is not replaced or reconstructed, patients typically recover function through physical therapy rather than needing an implant or prosthesis afterward.

Occipital-cervical Joint

The occipital-cervical joint is the craniovertebral articulation between the occipital condyles of the skull and the atlas, a specialized junction responsible for nodding motion of the head and structurally distinct from the intervertebral joints below it. Excision at this level is a high-stakes undertaking, generally reserved for resecting a tumor, an area of chronic infection, or abnormal bony or soft tissue that is causing instability or neural compression at the craniocervical junction. Because the vertebral arteries, upper spinal cord, and brainstem lie in immediate proximity, this procedure demands meticulous imaging-guided planning and is almost always paired with stabilization given the critical load-bearing and protective role of this junction. Documentation must clearly distinguish excision of joint or periarticular tissue from any accompanying fusion or arthrodesis procedure, as these are coded as separate root operations.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
