Sigmoid Colon
The sigmoid colon is the S-shaped, mobile segment connecting the descending colon to the rectum and is the most frequent site of diverticulosis, making it a common target for excision when diverticulitis, perforation, or a localized tumor requires segmental removal. Its redundant, freely mobile mesentery allows surgeons considerable latitude in mobilization, which facilitates both open and minimally invasive approaches, including robotic-assisted resection. Because the sigmoid sits adjacent to the bladder, ureters, and in women the uterus and adnexa, careful identification of the ureters is a standard step to avoid injury during dissection. When excision is performed for acute diverticulitis with abscess or fistula, the operative note often describes associated findings that, while clinically important, do not change the root operation from excision unless a full segment resection with anastomosis or diversion is instead performed.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
