Tibial Nerve
The tibial nerve is the larger terminal branch of the sciatic nerve, descending through the popliteal fossa and posterior compartment of the leg to supply the calf muscles and plantar sensation of the foot via the tarsal tunnel. Excision at this site is typically undertaken for a symptomatic benign tumor, most commonly a schwannoma presenting as a Tinel-positive mass in the popliteal region or medial ankle, or occasionally for a segment involved by malignancy. Because the tibial nerve is essential for plantarflexion and toe flexion as well as plantar sensation, surgeons aim to excise as limited a segment as possible and often send the specimen for intraoperative pathology to confirm margins. Documentation should specify the anatomic level, since proximal excisions near the popliteal fossa carry different functional consequences than those near the ankle.
Approach: Percutaneous Endoscopic
Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
