Taking or cutting out solid matter from a body part

Procedure Overview

Extirpation procedures remove solid material from a lower joint that does not belong there, such as loose cartilage or bone fragments, foreign bodies, blood clots, or broken pieces of prior hardware. A classic example is arthroscopic removal of a loose osteochondral fragment floating in the knee after an injury, or retrieval of a broken screw tip left in the ankle from an earlier surgery. Unlike excision, nothing that is normally part of the joint is being cut away; instead, something abnormal or displaced is being taken out.

These procedures are often performed to relieve locking, catching, or pain caused by loose material interfering with joint movement, or to clear debris that could otherwise cause ongoing irritation or infection. They range from minimally invasive arthroscopic retrieval to open exploration when the fragment is difficult to locate.

Patients generally undergo this procedure after trauma, a prior surgery that left residual material behind, or degenerative joint disease that has produced loose bodies within the capsule.

The left ankle joint shares the tibiotalar articulation's susceptibility to loose bodies, calcified debris, and infected synovial material, often arising from chronic anterior impingement, prior fracture, or osteoarthritic change. Extirpation addresses removal of that material through arthroscopic or, less commonly, open access, with the anterior working portals positioned carefully relative to the superficial peroneal nerve and extensor tendons to avoid iatrogenic injury. Unlike synovectomy or bony impingement resection, extirpation does not involve cutting or reshaping native joint tissue, only clearing what does not belong in the joint space. Because ankle arthroscopy frequently combines debridement of soft tissue with removal of discrete loose bodies, the operative note must specify which portion represents extirpated material to support accurate root operation selection.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

The documentation should clearly describe removal of solid matter that is abnormal to the joint - loose bodies, foreign objects, retained hardware fragments, or organized clot - as opposed to normal joint tissue. Operative notes using terms like loose body removal, foreign body retrieval, or debridement of a fragment support this code. A common coding error is applying Extirpation to routine synovial or cartilage debridement that is actually removal of native joint tissue, which belongs under Excision instead. Coders should also watch for cases where extirpation is performed alongside drainage of an infected or bloody joint, since the fluid and the solid material require separate root operations even within the same operative episode.

ExcisionExcision (0SB) is the key distinction: Excision removes a portion of the joint's own tissue, while Extirpation removes material that is not a normal part of the body part, such as a loose fragment or foreign object.
