Procedure Overview
Extirpation procedures on the upper arteries remove solid matter that does not belong in the vessel, most commonly blood clots, atherosclerotic plaque, or embolic debris obstructing flow through arteries of the chest, neck, shoulder, and arm. This is typically performed urgently when a clot or embolus is cutting off circulation to the brain, arm, or hand, causing pain, numbness, weakness, or threatening tissue loss.
The technique can be open, through a surgical incision that exposes the artery directly, or percutaneous, using a catheter threaded through the vascular system to retrieve or suction out the obstructing material. Because the arteries feeding the arms and neck also supply the brain in several cases, such as the carotid and vertebral origins, timely extirpation can be critical to preventing stroke or permanent limb damage.

The right common carotid artery ascends the neck from its origin at the brachiocephalic trunk to its bifurcation into internal and external branches, making it a frequent site for atherosclerotic plaque and, less commonly, embolic or thrombotic material. Extirpation at this level addresses obstructing debris found proximal to the bifurcation itself, distinguishing the procedure from endarterectomy of the bifurcation plaque complex, though the two are sometimes performed together.
