Extirpation procedures on the upper arteries remove solid matter that does not belong in the vessel, most commonly blood clots, atherosclerotic plaque, or embolic debris obstructing flow through arteries of the chest, neck, shoulder, and arm. This is typically performed urgently when a clot or embolus is cutting off circulation to the brain, arm, or hand, causing pain, numbness, weakness, or threatening tissue loss.
The technique can be open, through a surgical incision that exposes the artery directly, or percutaneous, using a catheter threaded through the vascular system to retrieve or suction out the obstructing material. The hand arteries are the small distal vessels continuing beyond the palmar arches to perfuse the digits and palm. Extirpation here is typically performed for an embolus or thrombus lodged in this narrow-caliber circulation, producing a cold, pale, or painful finger, and occasionally for calcified debris from connective tissue disease or trauma. Because these vessels are so fine, the offending material is usually retrieved by catheter-directed technique advanced from a more proximal puncture site under fluoroscopic guidance rather than open exposure. Given the delicate branching anatomy, identifying which digital or palmar branch is involved matters for accurate documentation, and laterality must always be recorded since the left and right hand circulations are coded as distinct body parts.
