Procedure Overview

Extirpation of the eye refers to the removal of solid matter, such as a foreign body, blood clot, or abnormal deposit, that does not belong in the eye but is not itself a piece of the eye's own tissue. This differs from cutting out a portion of an organ; instead, the surgeon is clearing out material that has become lodged or has accumulated, like a metallic fragment embedded in the cornea after an injury or a thrombus within an ocular vessel.

This type of procedure is often performed urgently, particularly for foreign body removal following trauma, since retained material can scar the cornea, trigger infection, or threaten vision if left in place. Other situations include clearing calcified deposits or removing a dislocated lens fragment that has become trapped within the eye.

In the left eye, the lens similarly functions to focus incoming light onto the retina, and extirpation applies specifically to retrieval of retained lens material, such as displaced nuclear or cortical fragments, that remains within the eye after a ruptured lens capsule from trauma or a complicated prior cataract procedure. These fragments can migrate posteriorly into the vitreous cavity, where they provoke inflammation, elevated intraocular pressure, or corneal endothelial damage if not extracted, often necessitating a pars plana vitrectomy approach for safe removal. This is distinct from routine cataract extraction of an intact or fragmented lens performed as the primary procedure, since extirpation here targets material left behind rather than the initial removal of the lens itself. Left-sided coding reflects the eye where the lens disruption originally occurred.

Approach: External

External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
