Procedure Overview

Extirpation removes solid material that doesn't belong in the respiratory tract - a foreign object a child inhaled, an inspissated mucus plug obstructing a bronchus, a blood clot, or a broncholith (a piece of calcified material eroding into an airway). The goal isn't to take a tissue sample or treat disease directly, but simply to physically take out an abnormal solid substance that is causing obstruction or infection risk.

Lung Lingula

The lung lingula is the projection of the left upper lobe that mirrors the right middle lobe in both function and vulnerability to recurrent infection, and it can accumulate necrotic tissue or abscess material when lingular pneumonia progresses to cavitation or when hematoma forms after trauma or prior surgery. Because the lingula is a relatively small and somewhat isolated segment of lung tissue, extirpation requires precise localization, often guided by imaging, to remove the solid material without unnecessarily disturbing adjacent left upper lobe parenchyma. This procedure is indicated when infected or necrotic debris fails to resolve with medical management alone. It is distinguished from lingulectomy in that only the obstructing or necrotic matter is removed, not the lingular tissue itself.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures.
