Extirpation in the lymphatic and hemic systems covers procedures that remove solid material that doesn't belong there, rather than removing a normal part of the body. Typical targets are a blood clot lodged in a lymphatic vessel, calcified debris in a lymph node, or an abscess pocket in the spleen. The material is taken out physically, by cutting, scraping, or suction, but the surrounding lymphatic structure itself is left in place.
This differs from taking a biopsy or removing an organ; the point of extirpation is clearing an obstruction or foreign substance so the vessel or organ can function normally again. It's often used when imaging shows a filling defect or mass that isn't a tumor to be resected but rather clotted or necrotic material to be evacuated. Recovery and follow-up depend on the underlying cause - a clot may signal a clotting disorder that needs separate treatment, while an abscess may require antibiotics afterward.

Mirroring the right side, the left axillary lymph nodes drain the left upper limb, lateral chest wall, and left breast, and they occupy the same tiered anatomic relationship to the pectoralis minor and axillary vessels. Extirpation of this group addresses solid matter that does not belong there and is not being taken for its own removal - examples include caseous material from granulomatous infection such as tuberculosis or cat-scratch disease, calcified nodal debris, or a retained foreign body. This differs fundamentally from an axillary dissection performed for breast cancer staging, where entire nodes are excised or resected. Because the left axilla lies near the same brachial plexus branches and thoracodorsal structures as the right, instrument access and nerve preservation are equally important considerations. Coders should confirm from the operative note that the intent was removal of abnormal material rather than nodal tissue sampling or complete node removal.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
