Procedure Overview

Extirpation procedures on the upper arteries remove solid matter that does not belong in the vessel, most commonly blood clots, atherosclerotic plaque, or embolic debris obstructing flow through arteries of the chest, neck, shoulder, and arm. This is typically performed urgently when a clot or embolus is cutting off circulation to the brain, arm, or hand, causing pain, numbness, weakness, or threatening tissue loss.

The technique can be open, through a surgical incision that exposes the artery directly, or percutaneous, using a catheter threaded through the vascular system to retrieve or suction out the obstructing material. Because the arteries feeding the arms and neck also supply the brain in several cases, such as the carotid and vertebral origins, timely extirpation can be critical to preventing stroke or permanent limb damage.

Radial Artery, Right

The right radial artery runs along the lateral forearm to the wrist and is widely used both as an access site for cardiac catheterization and as a bypass graft conduit, which makes catheter-related thrombosis one of the more common reasons for an obstructing lesion within it. Extirpation removes this thrombus, embolic material, or atherosclerotic debris from the arterial lumen, typically through a focused approach at the wrist or distal forearm, to preserve hand perfusion and the artery's potential future use for access or grafting. Because radial artery occlusion is usually well tolerated when the ulnar artery and palmar arch are intact, clinicians often confirm collateral circulation before or after the procedure, and the right-sided qualifier distinguishes it from the equivalent left-arm code.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

ExcisionExcision is the closest family conceptually, but the material removed there is native vessel tissue rather than an abnormal deposit like clot or plaque, so the distinguishing question is whether what came out was part of the artery or something foreign to it.
