Procedure Overview

Extirpation procedures on the upper arteries remove solid matter that does not belong in the vessel, most commonly blood clots, atherosclerotic plaque, or embolic debris obstructing flow through arteries of the chest, neck, shoulder, and arm. This is typically performed urgently when a clot or embolus is cutting off circulation to the brain, arm, or hand, causing pain, numbness, weakness, or threatening tissue loss.

The technique can be open, through a surgical incision that exposes the artery directly, or percutaneous, using a catheter threaded through the vascular system to retrieve or suction out the obstructing material. Because the arteries feeding the arms and neck also supply the brain in several cases, such as the carotid and vertebral origins, timely extirpation can be critical to preventing stroke or permanent limb damage.

Subclavian Artery, Left

The left subclavian artery branches directly off the aortic arch, distinct from its right-sided counterpart which originates from the innominate artery, and it supplies the left upper extremity along with contributions to the neck and chest wall through the vertebral and internal thoracic arteries. Extirpation performed here removes thrombotic or embolic material, or atherosclerotic debris, lodged within the vessel's lumen, which can occur in the setting of aortic arch pathology, subclavian steal physiology, or catheter-related thrombosis given this artery's frequent use for vascular access. Clearing this artery can be pertinent to both arm ischemia and vertebrobasilar symptoms because of its vertebral branch, and because the left subclavian artery is a common landmark in aortic arch procedures, precise documentation of the extirpation site relative to the aorta is important for accurate coding.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
