Procedure Overview
Extraction procedures pull or strip a portion or the entirety of an upper vein out of the body using physical force, most commonly performed as vein stripping for veins affected by chronic disease or as removal of an indwelling line-associated vein segment. Unlike a clean surgical cut, this technique threads an instrument along the length of the vein and draws it out through one or two small incisions, which is why it is described using force rather than sharp dissection.
This approach is generally reserved for superficial upper extremity veins that have become diseased, scarred, or are being removed to harvest the vessel for use elsewhere, such as when a segment of arm vein is taken for use as a vascular graft. The vein is not replaced after extraction; it is simply removed from its original location.

The right cephalic vein runs along the lateral arm from the wrist to the shoulder, where it drains into the axillary vein, and it is a workhorse vessel for peripheral IV access, blood sampling, and cephalic vein fistula construction for dialysis. Extraction applies when material such as an infected catheter segment, adherent thrombus, or fibrotic sheath must be physically pulled from within the vessel, as opposed to endovascular clot aspiration. Its superficial position along the lateral biceps groove generally permits a smaller incision than deeper veins, though repeated venipuncture or prior fistula surgery can leave the vessel scarred and more technically demanding to open. Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. Open is typical for procedures needing wide access, such as most laparotomies.
