Procedure Overview
Extraction in ophthalmic surgery describes pulling or stripping a body part out using mechanical force rather than a cutting or dissecting instrument. The best-known example is cataract extraction, where the clouded natural lens is drawn out of its capsule, though the term also applies to certain vitreous removal techniques where tissue is stripped away rather than incised.
This approach is chosen when tissue needs to be physically pulled free, often because it has become opaque, adherent, or otherwise dysfunctional and needs to be removed to restore or preserve vision. Cataract extraction remains one of the most commonly performed surgeries worldwide, typically followed by placement of an artificial lens to restore focusing ability.

Lens, Left
The left lens, held in place behind the iris by delicate zonular fibers, functions purely to focus light and has no independent blood supply, relying instead on the aqueous and vitreous for nutrition. Extraction here is most often performed to remove a cataractous lens whose opacification is impairing vision, with the lens content taken out of its capsular bag while the posterior capsule is typically preserved to support a subsequent intraocular lens implant. Zonular integrity determines how much support the capsular bag retains during removal, and weakened zonules from prior trauma or pseudoexfoliation increase the risk of complications such as posterior capsular tear or lens fragment displacement into the vitreous cavity. Because the lens has no capacity for regeneration, extraction is a definitive step that permanently alters the eye's native focusing mechanism.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

A coder should assign Extraction when documentation describes tissue being pulled or stripped out by force, most classically in lens extraction for cataract surgery, distinguishing the removal method from cutting-based approaches. The operative note should specify the technique, such as phacoemulsification followed by extraction of lens fragments, and identify whether an intraocular lens was subsequently inserted, which requires an additional code. A frequent error is failing to code the lens insertion separately when a cataract extraction is combined with prosthetic lens placement, since these are two distinct root operations performed in the same session.
