Procedure Overview

This family describes fluoroscopic imaging of the venous system, in which a continuous stream of X-rays is captured and displayed on a screen in real time, letting the physician watch contrast move through the veins as it happens. It is the imaging technique behind a traditional venogram, where an iodine-based contrast agent is injected into a vein and its path is tracked live to reveal narrowing, blockages, malformations, or the source of a leak.

It is commonly ordered to investigate deep vein thrombosis when other tests are inconclusive, to map veins before a surgical or interventional procedure, or to evaluate function of venous valves.

Inferior Vena Cava

The inferior vena cava is the largest vein in the body, formed by the confluence of the common iliac veins and returning blood from the lower body, pelvis, and abdominal organs to the right atrium. Fluoroscopic venography of the inferior vena cava is performed with contrast injected from a femoral or other lower extremity access point, commonly to evaluate thrombus extension from the lower extremities or pelvis, to confirm filter position before or after IVC filter placement, or to assess for external compression or congenital anomalies such as a duplicated or interrupted cava. Because the IVC is a frequent site for filter deployment to prevent pulmonary embolism, this imaging often serves a procedural guidance role in addition to diagnostic evaluation. As an unpaired midline structure, findings are documented for the vessel as a whole, with particular attention to segments above and below the renal veins since thrombus location relative to this landmark affects management.

Contrast: Other Contrast

Other Contrast captures imaging studies using a contrast medium that is neither high- nor low-osmolar iodinated agent, such as barium sulfate for gastrointestinal studies or gadolinium-based agents for certain applications outside standard MRI classification within this axis. It is used when the contrast administered does not fit the osmolarity-based iodinated categories, distinguishing it from those specific classes and from studies performed with no contrast.

Coders should look for language indicating live, moving imaging, such as "real-time," "under fluoroscopic guidance," or a description of contrast being tracked as it flows, to distinguish this from a single captured plain film. This family is most often confused with Veins Plain Radiography (B|5|0); the distinguishing feature is real-time, continuous imaging versus a single static exposure. It can also be mistaken for Veins CT Scan (B|5|2), but CT reconstructs a stack of individually acquired exposures into a computer-generated cross-sectional image rather than displaying one continuous live X-ray stream, and the two use different equipment and reporting conventions entirely.
